Ubuntu 12.04 software RAID has harddrive failure – how to fix it?

One of my machines is running Ubuntu 12.04 (LTS) with software (fake) RAID5. Unfortunately one of haddrives in my RAID5 setup died and I had to replace it with a new one. Btw. RAID5 configuration sustains failure of up to one harddrive without data loss.
